Eligiblity: The Applicant should be the resident of Chhattisgarh.

Required Documents:

1. Report from Patwari
2. Education certificate (class 5th/8th)
3. Self-Declaration
4. Current year B-1 form (If income is from agriculture)
5. Identification Proof (Voter’s Id/Adhaar Card/ Ration Card)
6. For Government servants; Original copy of pay slip issued by department/ For workers of Private company; Pay slip of the company.

Fee: रु. 30.00

Tenure: Normally the response will be given after 15 days of application

This Facility is also available to nearest CSC center at District collector Office, Tehsil office and Gram panchayat.
